Job Summary

Main Responsibilities
  • Assist the practice team in implementing effective management of patients and their needs.
  • Manage the nursing care of patients with chronic diseases, employing recall systems suitable to the practice needs.
  • Contribute to the development of policies, protocols, and guidelines within the practice.
  • Adhere to infection control measures within the practice according to local and national guidelines.
  • Aid the practice team in implementing new initiatives in the delivery of chronic disease management services.
  • Encourage the development of integrated services within Primary Care to include integrated practice-based teams and multidisciplinary working.
  • Recognize the opportunities for working in partnership with the Primary Healthcare team.
About Us

Jessop Medical Practice is a large practice with almost 17,000 patients, serving two surgeries located four miles apart. We are a CQC-rated Outstanding practice, utilizing the TPP SystmOne Clinical system. Our multi-disciplinary clinical team includes GP Partners, Salaried GPs, Advanced Clinical and Care Practitioners, Minor Illness Nurse Practitioner, and a Nurse Manager leading the Practice Nursing and HCA team.

Job Requirements
  • Clinical skills in wound management, management of long-term conditions, immunization, and vaccination.
  • Change management skills and ability to support patients to change lifestyle.
  • Good communication skills, both written and verbal.
  • Negotiation and conflict management skills.
Desirable Qualifications and Experience
  • Independent or supplementary nurse prescribing qualification.
  • Knowledge of SystmOne.
  • Cervical cytology.
  • Ear care.
