We are seeking a highly motivated and experienced Community Health Nurse to join our team at NELFT North East London Foundation Trust. As a Community Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes, residential care homes, and nurse-led clinics.
Key Responsibilities- Provide holistic health care to patients, families, and carers in their own homes, residential care homes, and nurse-led clinics.
- Work as a member of the integrated community health care team, collaborating with GPs, other professionals, and statutory and voluntary agencies.
- Deliver individualized care to patients under the supervision of the District Nurse.
- Initiate patient assessment and care planning with the support of a senior colleague.
- Work with patients and carers as partners in care.
- NMC Registration.
- Ability to work as an effective member of a multi-disciplinary Primary Health Care Team.
- Excellent verbal and written communication skills.
- Basic awareness of IT and IT skills.
- Able to move and handle patients in accordance with Organisation Manual handling policy and with sufficient mobility for a community role.
- Able to work with minimal supervision.
- Able to provide evidence-based care that is responsive to patient need.
- Able to use own initiative and make decisions.
- Mentor qualification.
- Relevant post-registration course in Wound management, diabetes, leg ulcer care, continence care, and assessment.
- Nurse Prescriber (V150).
- 1 year post-registration experience.
- Community Nursing Experience.
- Experience of teaching and assessing students and other members of staff.
- Experience of acting up in the absence of team leader in caseload and team management.
