We are seeking a skilled Health Visitor to join our team at Hunter AHP. As a Health Visitor, you will play a vital role in promoting health and wellbeing within the community. You will be responsible for managing caseloads and delivering the Healthy Child programme, ensuring exceptional care for families and individuals.
Key Responsibilities:
- Conduct maternal mood assessments and new birth visits to identify and address potential health issues
- Champion child safeguarding and promote overall wellbeing through effective collaboration with multi-agency teams
- Develop and implement strategies to improve health outcomes for families and individuals
Requirements:
- Postgraduate diploma or certificate in SCPHN Health Visitor or equivalent
- Current NMC registration as a Health Visitor
- Proficient in computerized recordkeeping and Excel
- Strong communication and interpersonal skills, with the ability to work effectively in diverse teams
- Commitment to continuous professional development and staying up-to-date with best practices in public health
