The Learning Disability Crisis Response Intensive Support Team provides intensive support to individuals with Learning Disabilities living in the community who are experiencing a period of crisis related to complex mental health and behavioral needs.
We offer rapid assessment, support, and risk management to prevent placement breakdown and inappropriate psychiatric hospital admissions.
The team consists of Learning Disability and Mental Health Nurses, Support Workers, Speech and Language Therapists, Occupational Therapists, Psychologists, and Psychiatrists. We provide a service 7 days a week, from 8 am to 9 pm, including bank holidays.
Main Duties and Responsibilities- Provide oversight and governance to the clinical and operational systems and standard operational procedures in the team to ensure patient referrals are triaged, risk assessed, risk managed, and allocated for further interventions.
- Provide clinical supervision to Band 6 practitioners to ensure the delivery of high-quality clinical care pathways and hold oversight for the supervision of other team members.
- Hold a small caseload and ensure the Trust's clinical and operational policies and procedures are adhered to and implemented within the team.
- Ensure the delivery of a clinically effective service that meets the needs of individuals, families, and communities in line with local and national development plans, Trust policies, and procedures.
- Carry 24-hour responsibility and professional and managerial accountability, including management of staff, budgets, staffing rotas, quality, and standards of practice of all team members.
- Have overall responsibility to ensure a personalized service to clients/service users is delivered in accordance with LPT Trust standards.
- Provide good leadership and management to develop and sustain a positive working culture.
- Ensure staff are trained appropriately to competently deliver the care required and ensure supervision and mentorship is in place.
- Be influential within the organization and provide detailed advice to others internally and externally to the organization, within their sphere of competence.
