About the Role
We are seeking a highly skilled Clinical Pharmacist to join our team at North Swindon Practice. As a key member of our pharmacy team, you will play a pivotal role in improving the quality of care and operational efficiency within our primary care network.
Key Responsibilities
- Work closely with our Senior Clinical Pharmacist to develop and run processes for medicines management on transfer of care and systems for safer prescribing.
- Provide pro-active support for effective medicines management for patients suffering from chronic diseases.
- Manage medication for patients with common minor ailments, within the limits of professional competency.
- Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
- Provide primary support to practice staff regarding prescription and medication queries.
- Support the practices repeat prescribing systems and deal with acute prescription requests and medicines reconciliation on transfer of care.
- Implement systems for safer prescribing, providing expertise in clinical medicines advice.
- Provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
- Ensure that the practices integrate with community and hospital pharmacies to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
Person Specification
- Masters degree in Pharmacy (MPharm) or equivalent Post graduate diploma or equivalent training/experience. Independent Prescriber (or interest in working towards this qualification).
- Work as part of a multi-disciplinary primary healthcare team in both a back office and potential patient-facing role.
