Job Summary

We are seeking a skilled and compassionate Health Aide/PCA to join our team at East Boston Neighborhood Health Center. As a Health Aide/PCA, you will provide essential care and support to our participants in their homes, promoting their independence and well-being.

Key Responsibilities
  • Monitor and implement participant care plans, ensuring timely and effective delivery of services.
  • Document care provided and report any changes or concerns to the team in a timely manner.
  • Assist participants with daily living activities, such as toileting, bathing, and dressing.
  • Accompany participants to appointments and provide transportation as needed.
  • Perform simple treatments under the direction of nursing staff, including catheter care and skin assessments.
Requirements
  • High School graduate or equivalent required.
  • Current certification as a H.H.A. /C.N.A. from an approved program.
  • A minimum of one year experience working with a frail or elderly population.
  • Prior experience as a health aide, personal care assistant, or similar role.
  • Excellent customer service and communication skills; bilingual a plus.
  • Physical capability to provide ADL and mobility assistance to all PACE participants.
