About the Role

We are seeking an experienced Support Worker to join our Home First Team in Kent. As a Support Worker, you will play a vital role in providing short-term, personal care and rehabilitation to support patients to return home following a hospital admission.

Key Responsibilities

  • Deliver high-quality care and support to patients in their own homes, promoting independence and well-being.
  • Work collaboratively with the multidisciplinary team to ensure seamless care transitions and effective discharge planning.
  • Conduct comprehensive assessments of patients' needs, identifying areas for support and developing personalized care plans.
  • Provide emotional support and guidance to patients and their families, promoting a positive and empowering experience.
  • Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes.

Requirements

  • Proven experience as a Support Worker or Community Health Care Assistant in a similar setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, prioritizing tasks and managing time effectively.
  • Basic IT skills and proficiency in using electronic patient records and other relevant software.
  • Full UK driving license (or willingness to obtain) and access to a vehicle for work-related travel.
