Position Overview

The Hospice at Home initiative provides compassionate care for individuals in their final stages of life, allowing them to remain in the comfort of their own homes. Our approach is comprehensive, ensuring that care is customized to meet the unique needs of each patient. Our dedicated team of Nurses and Healthcare Assistants deliver both practical and emotional support to patients and their families.

We are currently seeking a Band 6 Community Nursing Lead to join our team in a permanent capacity. This full-time position will be based at our treatment center while serving patients throughout the region.

Key Responsibilities

The Community Nursing Lead will play a vital role within the Hospice at Home team, providing specialized knowledge and skills in palliative and end-of-life care. The individual will ensure that the highest standards of care are delivered to patients and their families in their homes. Responsibilities include:

  • Conducting comprehensive assessments, planning treatments, and evaluating patient care.
  • Maintaining and promoting high standards of care in accordance with organizational policies.
  • Mentoring and supervising junior staff members, including Bands 2, 3, and 5.
  • Coordinating and managing Healthcare Assistant teams to ensure adequate coverage across the service.
  • Collaborating with Community Palliative Clinical Nurse Specialists to assess patients in their homes as needed.

To excel in this role, candidates must be registered with the NMC as a Nurse and possess a relevant postgraduate qualification or equivalent experience, along with a commitment to ongoing professional development. Previous experience in a Band 5 role and a solid understanding of palliative and end-of-life care are essential.

Due to the nature of this position, a valid UK driving license and access to a vehicle for work purposes are required.

Established in 1981, Arthur Rank Hospice Charity is dedicated to supporting individuals in the region facing serious illnesses or life-limiting conditions, as well as those requiring end-of-life care. We provide care to approximately 3,600 patients annually, both at our treatment center and in their homes, at no cost to them.

Qualifications and Experience

Essential:

  • Registered General Nurse.
  • First-level degree in a related field.
  • Relevant postgraduate qualification or equivalent experience.
  • Evidence of continued professional development.
  • Experience as a practice supervisor or assessor, supporting learners in accordance with NMC standards.

Desirable:

  • Experience in palliative and end-of-life care.
  • Teaching experience.
  • Experience in multi-professional collaboration.

Other Requirements

Essential:

  • Valid UK driving license and access to a vehicle for work purposes.
