We are seeking a highly experienced and skilled professional to lead our housing and care integration efforts. As the Head of Housing and Care Integration, you will play a critical role in developing and implementing strategies to improve housing outcomes for our patients and clients.
Key Responsibilities- Lead the development of a comprehensive housing strategy that aligns with the Trust's goals and objectives.
- Collaborate with stakeholders across the health and social care sector to identify and address housing needs and barriers.
- Develop and maintain strong partnerships with housing providers, local authorities, and other key stakeholders to ensure effective delivery of housing services.
- Provide leadership and guidance to teams working on housing-related projects and initiatives.
- Monitor and evaluate the effectiveness of housing programs and services, making recommendations for improvement.
- Represent the Trust in housing-related forums and meetings, advocating for the needs of our patients and clients.
- Master's degree in a relevant field, such as housing, social work, or healthcare.
- Significant experience in housing and care integration, preferably in a leadership role.
- Proven track record of developing and implementing effective housing strategies and programs.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with stakeholders.
- Ability to think strategically and make informed decisions.
- Knowledge of housing laws, policies, and regulations.
