Integrated Primary Care Network/Hospital Pharmacist

We are seeking a dynamic and enthusiastic clinical pharmacist to join our team at Wirral University Teaching Hospital NHS Trust. As a key member of our Primary Care Network, you will play a vital role in delivering safe, effective, and patient-centered care to our local population.
Key Responsibilities- Support the delivery of medicines optimisation services within the Primary Care Network, reducing waste and maximising patient outcomes through structured medication review.
- Contribute to the development of new ways of working and using your skills to evolve PCN services.
- Promote good practice in line with clinical and operational guidelines, and clinically support pharmacy technicians with medicines reconciliation post-discharge and other medicines projects.
- Build strong relationships with community pharmacy and trust pharmacy colleagues, and work collaboratively to improve patient care.

Person Specification- Vocational Master Degree in Pharmacy plus 1 year pre-registration experience.
- Postgraduate Pharmacy qualification or equivalent vocational experience.
- Post-registration clinical experience in a relevant area of practice.
- Significant relevant and ongoing CPD.
