The Role: Support Worker
As a Support Worker at Connected Health, you'll play a vital role in enhancing the well-being of our clients. Your days will be diverse and rewarding, as you assist individuals with their daily living needs, tailored to their unique requirements.
Key Responsibilities:- Provide personalized support with daily tasks, collaborating closely with service users, families, and professionals involved in their care plans.
- Adhere strictly to Connected Health's policies and procedures, ensuring a safe and supportive environment for all.
- Work within established care and risk assessments, tailoring your approach to meet the specific needs of each individual.
- Conduct regular health and safety checks, upholding our commitment to a secure living space.
- Demonstrate confidence in lone working, adhering to established risk assessments and protocols.
- Ensure safe medication administration and ordering, prioritizing the well-being of our clients.
- Empower service users by respecting their preferences and promoting independence in all aspects of their lives.
- Utilize de-escalation techniques to manage challenging behaviors effectively and respectfully.
- Maintain accurate daily notes, handover reports, and documentation, ensuring seamless communication between shifts.
- Collaborate with the multidisciplinary team (MDT) to provide holistic care that addresses both emotional and physical needs.
- Build strong professional relationships with service users, families, and colleagues, fostering a positive and supportive environment.
