Position Overview

The City Health Care Partnership CIC is dedicated to providing exceptional healthcare services to the community. We are seeking a committed and experienced General Practitioner with an extended role to join our innovative team. This position focuses on delivering tailored care to older adults and frail patients, ensuring they receive the highest quality of support in their preferred environments.

Key Responsibilities

In this dynamic role, you will engage in various responsibilities, including:

  • Conducting thorough Geriatric Assessments within community settings.
  • Providing medical assistance to community-based units aimed at preventing hospital admissions and facilitating safe discharges.
  • Offering expert advice and guidance to healthcare professionals involved in the care of older patients.
  • Responding to urgent community needs within a two-hour timeframe.
  • Supporting patients with acute requirements through a virtual ward system.

This is a remarkable opportunity for a General Practitioner who is passionate about geriatric care and eager to contribute to the development of a prominent community service. You will collaborate closely with a multidisciplinary team to assess and proactively manage the care of older individuals, ensuring they receive comprehensive and holistic support. Qualifications and Experience

Essential Qualifications:

  • Full and current GMC registration.
  • Medical degree.
  • Postgraduate qualification in geriatric care or a commitment to pursue one.

Essential Experience:

  • Post-registration training in geriatric medicine.
  • Experience in supervising clinical staff to uphold clinical standards.
  • Evidence of ongoing professional development.
  • Experience in conducting clinical audits.

Knowledge and Skills

Essential Knowledge:

  • Up-to-date understanding of guidelines in geriatric medicine.
  • Knowledge of evidence-based practice and clinical effectiveness.
  • Understanding of clinical governance principles.
  • Ability to utilize clinical evidence to inform decision-making.
