About the Role

We are seeking a highly skilled and experienced Respiratory Physiotherapist to join our Community Respiratory Team at Livewell Southwest. As a key member of our team, you will play a vital role in supporting the management of chronic lung conditions and preventing hospital admissions through assessment, support, education, and monitoring of patients with complex respiratory conditions in their own homes.

Key Responsibilities
  • Provide specialist physiotherapy assessment and treatment for patients with highly complex respiratory conditions, including those with advanced COPD and comorbidities.
  • Lead pulmonary rehabilitation groups across the catchment area, promoting understanding and adherence to treatment plans.
  • Collaborate with multidisciplinary teams to develop comprehensive management plans, incorporating evidence-based practice and national guidelines.
  • Deliver formal teaching sessions to patients, pulmonary rehabilitation groups, and multidisciplinary teams, promoting education and empowerment.
  • Provide specialist advice and support to colleagues, promoting a culture of collaboration and shared learning.
  • Participate in service development, exploring innovative practice and contributing to the growth of our Community Respiratory Team.
Requirements
  • Degree or equivalent in physiotherapy, with HCPC registration.
  • Experience in managing patients with a range of respiratory conditions, including autonomous practice and caseload management.
  • Excellent verbal, non-verbal, and written communication skills, with the ability to teach and communicate effectively with patients, carers, and multidisciplinary teams.
  • Ability to work autonomously, prioritizing tasks and managing time effectively in a fast-paced environment.
  • Enhanced disclosure and barring service (DBS) check satisfactory to the organization.
