Job Summary

This is an exciting opportunity to support the safe and rapid discharge of service users who no longer require a hospital setting. The Manchester Care Group has developed a multi-disciplinary integrated discharge team to facilitate timely discharge from inpatient units across the locality and wider GMMH footprint.

Main Duties

The dual diagnosis worker will specialise in the facilitation of a co-occurring conditions clinical model, offering dual assessment, formulation, brief intervention, extended intervention, and planning. This will occur during inpatient stay or as part of the discharge plan and handover to community mental health and/or substance misuse services.

About the Role

This role will enhance pathways to substance use services and support continuity of care between inpatient and community services. The post holder will lead on facilitating the discharge of complex cases and develop excellent working relationships with external partners to progress barriers in relation to service users deemed clinically ready for discharge.

Key Responsibilities
  • Delivery of dual diagnosis case advice, in line with national best practice guidance, NICE, and Department of Health guidance, Trust policy, and in consultation with other dual diagnosis colleagues.
  • Collaboration with colleagues and service leaders to agree and implement dual diagnosis plans that address the development and sustenance of high-quality practice within the service.
  • Implementation of harm minimisation measures to ensure safe environments and minimising substance use on Trust premises.
  • Carrying a small, specialised caseload of clients with complex dual diagnosis disorders, including clients presenting with high levels of risk and co-morbidity.
  • Development of dual diagnosis champions, the dual diagnosis group work initiative, and policy and practice within host wards.
Person Specification

Qualifications

  • Relevant professional qualification (RMN, Social Work)
  • Psychosocial Intervention qualification cert in MI / diploma in CBT

Experience and Knowledge

  • Experience of working with service users in a mental health setting
  • Experience of assessing service users' needs and involving them in the planning of their care
  • Knowledge of the physical, psychological, and social effects of substance misuse and appropriate interventions
  • Understanding of current national policy and initiatives in relation to working in the mental health and substance misuse field
  • Understanding of the complex needs of this client group
