Procare Health Limited is seeking a dedicated Patient Care Navigator to enhance our healthcare team. As we expand our Primary Care Network (PCN), this role is essential in assisting our community and fostering the health and wellness of our patients.
Patient Care Navigators play a vital role in identifying and collaborating with individuals, particularly the elderly and those with chronic conditions, to deliver proactive coordination and guidance of care across various health and social services.
In this position, you will work closely with healthcare professionals to manage a patient caseload, serving as a key contact to ensure that patients and their caregivers receive the necessary support. Your responsibilities will include helping patients understand and manage their health conditions while addressing their evolving needs.
The ideal candidate will possess a caring, reliable, and patient-centered approach, enjoying interactions with diverse populations. Strong written and verbal communication skills, along with excellent organizational and time management abilities, are essential. You should be motivated, proactive, and adaptable, eager to collaborate within a team and committed to delivering high-quality support to patients and their families.
This role is integral to our multidisciplinary team, working alongside various professionals to provide tailored care and promote a personalized care philosophy.
Main Responsibilities
As a Patient Care Navigator, you will:
- Support the transition from reactive to proactive care models within the PCN, identifying needs and coordinating personalized care for our patient population.
- Assess patient needs and facilitate access to necessary services and support, collaborating with Social Prescribing Link Workers, Health Coaches, and other professionals as appropriate.
- Assist patients in preparing for clinical discussions with healthcare providers, empowering them to take an active role in managing their health and making informed choices.
- Enhance the quality of life for individuals by providing guidance and support tailored to their unique circumstances.
