Position Overview

As we expand our Primary Care Network (PCN), Procare Health Limited is seeking a dedicated Patient Care Navigator to enhance our existing team. This role is pivotal in supporting our community and enhancing the health and wellbeing of our patients.

Patient Care Navigators play a crucial role in identifying and assisting individuals, particularly the elderly and those with chronic conditions, by providing proactive coordination and guidance through health and care services.

Working closely with healthcare professionals, the Patient Care Navigator will manage a caseload of patients, serving as a central point of contact to ensure they and their caregivers receive the necessary support. This includes helping patients understand and manage their health conditions while addressing their evolving needs.

The ideal candidate will embody compassion, reliability, and a patient-centered approach, enjoying interactions with a diverse range of individuals. Strong written and verbal communication skills, along with excellent organizational and time management abilities, are essential. A proactive and motivated attitude, coupled with a willingness to learn and collaborate within a team, is crucial for delivering high-quality support to patients and their families.

This role will be an integral part of our multidisciplinary team, collaborating with Social Prescribing Link Workers and other professionals to provide tailored care and promote a personalized care approach.

Main Responsibilities

Patient Care Navigators will assist the PCN and practice in transitioning from a reactive care model to a proactive one, identifying needs and coordinating personalized care for our population.

Key responsibilities include:

  • Reviewing patient needs and facilitating access to necessary services and support.
  • Collaborating with Social Prescribing Link Workers, Health and Wellbeing Coaches, and other professionals to ensure comprehensive care.
  • Supporting patients in preparing for and following up on clinical discussions with primary care professionals.
  • Empowering individuals to enhance their quality of life through effective care management.
