Key Responsibilities- Assist in the delivery of tasks based on customers' personal support plans, needs, and aspirations.
- Work closely with external agencies and professionals to assess and engage support for our customers.
- Empower customers to develop independent living skills and support them in achieving their goals.
- Collaborate with colleagues to ensure the smooth delivery of support and create support plans with customers.
- Carry out risk assessments, support planning, goal setting, and regular reviews.
- Experience in safeguarding, risk assessments, confidentiality, support plans, and working collaboratively.
- Ability to work on your own initiative and positively influence people from diverse backgrounds.
- Previous experience in a similar environment, with a focus on supporting vulnerable individuals.
- Valid driving license and access to a vehicle insured for business purposes.
- Enhanced DBS check (paid for by Home Group Limited).
- Competitive hourly rate of £12.61 (pro rata) with great benefits, including a Health Cash Plan.
