We are seeking a highly skilled and experienced Social Worker to join our Integrated Learning Disability Team at Livewell Southwest. As a key member of our team, you will be responsible for providing high-quality social care services to individuals with complex needs.
Key Responsibilities- Discharge statutory social care functions on behalf of the organisation and uphold the standards outlined in the Professional Capabilities Framework for Social Work.
- Undertake complex and proportionate assessments that are evidence-based, including Mental Capacity assessments and Care Act assessments.
- Arrange and review the delivery of social care support, promoting the health, safety, and wellbeing of individuals.
- Support individuals to take risks in complex situations, promoting their outcomes and wishes.
- Undertake safeguarding investigations and consider decisions made in a person's best interest that may deprive them of their liberty.
- A qualification in Social Work, such as a postgraduate diploma, bachelor degree, or masters level.
- Proven relevant experience of working in the health and social care sector.
- Knowledge of social work theories and models, the strengths and limitations, and how they can be applied to practice.
- Knowledge of the legal framework relevant to social work, including the Care Act (2014), Mental Capacity Act (2005), Mental Health Act, and The Human Rights Act.
- Knowledge of the Professional Capability Framework and the standards set by the social work regulatory body, and maintaining professional registration.
- Any postgraduate qualifications relevant to social work, such as a Graduate Certificate in Practice Education or Approved Mental Health Professional.
- Experience of working with other professionals in an integrated service.
