Main Responsibilities:

  • Develop and implement health promotion and wellbeing programs to improve patient outcomes.
  • Manage patients with a range of acute and chronic medical conditions, providing expert advice and care.
  • Implement and evaluate individualized treatment plans for patients with chronic diseases.
  • Identify and manage patients at risk of developing long-term conditions, preventing adverse effects on their health.
  • Provide advanced, specialist nursing care to patients as required, in accordance with clinical evidence and best practice.
  • Provide wound care, including ulcer and Doppler services.
  • Provide specialist clinics, such as ear syringing and contraception services.
  • Collect and process pathological specimens.
  • Provide travel medicine services.
  • Request pathology services as necessary.
  • Process and interpret pathology and other test results.
  • Deliver chronic disease clinics, providing patient care and referring patients to secondary or specialist care as required.

Person Specification:

  • Registered Nurse with the Nursing and Midwifery Council.
  • Qualified Independent Nurse Prescriber on the NMC register.
  • Masters degree required for qualification post December 2020.
  • Minor illness qualification.
  • Experience of working as a practice nurse or community nurse.
  • Wound care and removal of sutures and staples experience.
