Job Overview
- To provide high-quality care and support services to our service users, in accordance with their individual needs and preferences.
- To work collaboratively with our multidisciplinary teams to ensure seamless care delivery and maximize opportunities for our service users.
- To maintain accurate and up-to-date records of service user information, care plans, and other relevant documentation.
Key Responsibilities
- Deliver person-centered care and support services that meet the individual needs and preferences of our service users.
- Support service users to maintain their independence and well-being, and to engage in activities that promote their physical and mental health.
- Work collaboratively with other healthcare professionals to ensure seamless care delivery and maximize opportunities for our service users.
- Contribute to the development and implementation of care plans that meet the individual needs and preferences of our service users.
- Provide emotional support and companionship to our service users, and to promote their dignity and respect.
Requirements
- Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and other healthcare professionals.
- Ability to work collaboratively as part of a multidisciplinary team, and to contribute to the development and implementation of care plans.
- Knowledge of care and support principles, and the ability to apply this knowledge in practice.
- Ability to maintain accurate and up-to-date records of service user information, care plans, and other relevant documentation.
- Commitment to delivering high-quality care and support services that meet the individual needs and preferences of our service users.
