Position Overview
County Durham and Darlington NHS Foundation Trust stands as one of the most extensive integrated healthcare providers in the region, delivering both hospital and community services to a diverse population exceeding 650,000 individuals. Our organization is characterized by ambition and innovation, supported by a dedicated workforce of over 7,000 professionals. Our guiding principle is to ensure care is delivered 'Right First Time, Every Time', with a commitment to providing safe, compassionate, and coordinated care to the communities we serve.
Are you ready to embrace new challenges within our progressive Trust?

Are you passionate about delivering comprehensive, patient-centered care as part of a dynamic multidisciplinary team?
We are seeking a motivated Advanced Physiotherapy Practitioner to become a vital member of our Community Therapy Team. The successful candidate will engage with various community settings, including rehabilitation facilities, and will work with patients recovering from falls, amputations, strokes, and neurological conditions.
Key Responsibilities
The selected candidate will autonomously manage a diverse caseload, including patients with complex needs, and will be responsible for conducting specialized assessments and developing treatment plans for adults referred to our team. This role involves addressing a variety of physical impairments and disabilities resulting from injury, illness, surgery, or chronic conditions, all within community environments.
Opportunities include:
- Enhancing skills in holistic assessments that extend beyond traditional physiotherapy practices.
- Developing leadership and service enhancement capabilities.
- Taking charge of a specific area of expertise within the service, working independently.
- Mentoring junior staff and contributing to their professional growth.
- Participating in peer support and service training initiatives.
- Engaging across multiple community sectors, including rehabilitation, stroke recovery, and neurotherapy.
About Our Organization
We encourage all candidates to accept an interview slot in our system, even if previously arranged with a manager. It is essential to present all required certifications as outlined in the person specification to proceed with pre-employment checks.
Our services are delivered from two main acute facilities, alongside community hospitals and numerous other settings, ensuring comprehensive care is accessible to our patients.
We actively encourage applications from individuals with disabilities and those from Black, Asian, and Minority Ethnic (BAME) backgrounds, as these groups are currently underrepresented in our workforce.
Job Responsibilities
Organize and prioritize your workload while overseeing the clinical activities of those you supervise within a designated area.
Independently assess, prioritize, and plan interventions for both routine and urgent referrals received by the community therapy services.
Utilize sound clinical judgment to prioritize patient care in a demanding and unpredictable environment, ensuring optimal resource utilization.
Assist supervised staff in managing their caseloads and provide guidance on appropriate interventions for complex cases on a daily basis.
Conduct comprehensive and holistic assessments of patients, including those with diverse or complex presentations and multiple pathologies.
Employ analytical and clinical reasoning skills to accurately diagnose conditions, selecting appropriate treatments and implementing rehabilitation programs.
Collaborate with the multidisciplinary team to facilitate patient-centered goals.
