Key Responsibilities:
  • Provide high-quality information, advice, and signposting to young people in need.
  • Process and triage referrals from various sources, ensuring timely and effective support.
  • Conduct comprehensive assessments to identify housing and support needs, and develop personalized plans to prevent homelessness.
  • Work closely with young people, their families, and external agencies to ensure seamless support and advocacy.
  • Maintain accurate and professional case records, ensuring data integrity and compliance with regulations.
  • Collaborate with internal and external partners to provide a holistic response to young people's housing and support needs.
  • Develop and maintain effective relationships with young people, their families, and external stakeholders.
Requirements:
  • GCSE English and Maths (C grade or above) or equivalent.
  • Essential experience working with young people in a similar setting, delivering frontline triage or assessment services.
  • Knowledge of housing rights, welfare reform, and relevant legislation.
  • Commitment to anti-discriminatory practice and ability to work with diverse groups.
Desirable:
  • NVQ 3 Advice and Guidance, Health and Social Care, or equivalent.
  • Housing qualification or experience.
  • Ability to work with figures and maintain high standards.
