Job Title: Homeless Prevention Specialist

Reports to: Homeless Prevention Coordinator

Salary: Scale 6, Points 26-28, £24,744 - £26,398 per annum, pro rata

Hours of work: Full Time (37.5 hours per week)

Key Responsibilities:

ASSESSMENT/SUPPORT

  • To provide high-quality information, advice, and signposting to young people in need.
  • To process and triage referrals from various sources, ensuring timely and effective support.
  • To conduct comprehensive assessments of housing and support needs, identifying risks and developing strategies to prevent homelessness.
  • To investigate housing and personal histories to determine eligibility, priority need, and local connection.
  • To maintain accurate and professional electronic case records, ensuring data integrity and compliance.
  • To provide homelessness and housing options to young people, meeting their needs and managing expectations.
  • To make decisions in line with legislation, ensuring adherence to the Housing Act 1996 and the Homelessness Reduction Act 2017.
  • To present cases to the local authority in a timely manner, ensuring effective collaboration.
  • To manage a caseload of young people through the prevention and relief pathways.
  • To build and maintain effective working relationships with internal and external partners.
  • To provide a holistic response to young people's housing and support needs.
  • To develop and maintain Personalized Housing Plans (PHPs), ensuring young people receive relevant support and services.

HOME VISIT & MEDIATION

  • To undertake mediation sessions with young people and their families, improving relationships and preventing homelessness.
  • To utilize assessment tools to identify needs and develop evidence-based plans.
  • To conduct home visits and accompany young people to appointments, as needed.
  • To plan, implement, review, and evaluate interventions to achieve positive outcomes.

ADVOCACY

  • To promote young people's understanding of assessment and application processes, ensuring they are supported throughout.
  • To advise and advocate on benefits, specialist support services, and housing rights, empowering young people to access services.

QUALIFICATIONS

Essential:

  • GCSE English and Maths – C Grade and above or equivalent.

Desirable:

  • NVQ 3 Advice & Guidance, Health & Social Care, or equivalent, or working towards Youth Work, Social Work, or equivalent.
  • Housing qualification.

ROLE EXPERIENCE

Essential:

  • Experience working with young people in various settings, including Youth Work, Residential, Social Work, Probation, Community Work, Education, and Training.
  • Delivering frontline triage or assessment services.

Desirable:

  • Working with volunteers or students.

KNOWLEDGE:

Essential:

  • Housing Rights and Welfare Reform Knowledge.
  • Housing Legislation, including the Housing Act and Homelessness Reduction Act.
  • Knowledge of the Children's Act, including Sections 17, 20, and 47.
  • Commitment to Anti-Discriminatory practice.

Desirable:

  • Social Skills and Job Training Skills.
  • Ability to work with figures.
  • Ability to maintain and improve standards.
