Community Respiratory Care Nurse

Position Overview
We are pleased to announce a rewarding opportunity to join our Community Respiratory Service as a Community Respiratory Care Nurse.
The successful candidate will play a vital role in enhancing the community respiratory service, delivering thorough, skilled, and evidence-based respiratory care to patients within the primary care environment.
You will be instrumental in providing proactive patient care, facilitating appropriate admission avoidance, and promoting early discharge, while fostering collaborative relationships with Community Nursing, Primary Care, Social Care, and the Acute Trust.
As part of a well-established clinical team, you will deliver respiratory care across the region, providing support to local hospitals and working within a multidisciplinary team. Our Community Respiratory Service is fortunate to include Pulmonary Rehabilitation and Respiratory Physiotherapy, making this an exciting time to contribute to the expansion of our community services.
Key Responsibilities
Serve as a proficient practitioner, showcasing respiratory clinical expertise while offering specialized guidance and developing training programs for the multidisciplinary team. Assist in creating and implementing patient and caregiver education initiatives to enhance self-management. Act as a deputy for the Respiratory Team Lead when necessary. Conduct comprehensive assessments and examinations of patients, including those with complex presentations or multiple health issues, utilizing advanced assessment techniques and tailoring treatment plans to meet individual patient needs. Engage in multidisciplinary team meetings to discuss the ongoing management of respiratory patients, particularly those with intricate needs. Ensure that patients receive evidence-based care, continuously evaluating their progress and self-management strategies.About Our Organization

Qualifications and Training
Essential Requirements
Registered General Nurse Nurse Prescriber (or willingness to pursue training) Evidence of recent training or qualifications in respiratory care, such as a diploma in respiratory care or COPD.Experience
