Your Role
We are seeking a dedicated and skilled Practitioner in Social Care who is passionate about enhancing individuals' initial interactions with Adult Social Services.
Key Responsibilities:
- Assess and prioritize incoming requests to ensure individuals receive timely and appropriate assistance.
- Address both mandatory and voluntary safeguarding concerns and facilitate collaborative meetings to assist individuals in managing risks.
- Manage a limited, short-term caseload for evaluations in accordance with the Care Act 2014.
- Engage in networking and multi-agency collaboration, demonstrating confidence in working with individuals facing challenges while exhibiting flexibility, resilience, and creativity.
- Empower individuals by focusing on their strengths and available resources.
- Provide Social Care expertise to the broader Multi-Disciplinary Team.
About You
As a seasoned Practitioner in Social Care, you will have successfully completed an ASYE program and be on a path towards achieving a progressive level within the field.
In this role, you will collaborate with colleagues to shape, enhance, and assess the effectiveness of our Front Door Service. We are committed to investing in our team members through regular supervision, peer support, and a comprehensive training and development plan.
