Job Title: Nurse Practitioner

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at The Lakenham Surgery. As a key member of our primary care team, you will be responsible for providing high-quality patient care, working autonomously and as part of a multidisciplinary team.

Job Summary:

The successful candidate will have a strong background in nursing, with a focus on primary care and a passion for delivering exceptional patient outcomes. You will be responsible for managing individual patient consultations, assessing and diagnosing a wide range of health care conditions, and providing treatment and care as needed.

Main Duties:
  • Manage individual patient consultations, including history taking, clinical assessment, and diagnosis.
  • Assess and diagnose across a wide range of health care conditions, order investigations, and plan and provide treatment as needed.
  • Work directly and independently with patients, diagnosing, initiating, and concluding treatment, including prescribing within appropriate professional scope and competencies.
  • Use SystmOne and Ardens Templates for recording of consultations and clinical administration, demonstrating good practice in clinical note making.
  • Adhere to the Nursing & Midwifery Council (NMC) Code of Professional Conduct and be familiar with The Scope of Professional Practice and other NMC advisory papers.

The Lakenham Surgery is a long-established surgery based just outside the centre of Norwich, serving a diverse patient population of approximately 8,900 patients. We aim to provide positive access to services and high standards of clinical care, and we are expanding our team to further support these objectives.

Person Specification:

Essential:

  • Nurse Practitioner qualification (including MSc)
  • Independent & Supplementary Prescriber qualifications
  • Registration with NHSBSA as a Prescriber
  • Eligibility to work as a Nurse in the UK and with UK current work permit if required
  • English language skills sufficient to enable positive and effective verbal and written communication about medical topics with patients and colleagues
  • RGN with current NMC registration
  • Ability to work as part of Enhanced Access service provision

Desirable:

  • Experience in the management of Long Term Conditions such as Diabetes, Asthma, COPD, Hypertension & CHD
  • Familiarity with a wide range of nursing procedures
