About the Role

We are seeking a highly skilled and experienced Consultant Advanced Clinical Practitioner to join our West Kent Home Treatment Service. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the development of our services.

Key Responsibilities
  • Provide expert clinical leadership and guidance to the team, ensuring excellence in clinical practice and patient care.
  • Develop and implement strategies to improve patient outcomes and service delivery, working closely with colleagues and stakeholders.
  • Contribute to the development of education and training programs, sharing your expertise with colleagues and trainees.
  • Collaborate with national and international research networks, contributing to the development of a centre of excellence in care and treatment for older people.

Requirements
  • First-level registration with NMC, HPCP, or GPHC.
  • Minimum of 5 years post-registration experience, with experience at band 8a or above.
  • Master's level or higher degree.
  • Teaching qualification.
  • Research qualification.
  • Leadership experience.
  • Non-medical prescribing experience.
Desirable Qualifications and Experience
  • Psychology or counselling experience and training/education.
  • Extensive clinical experience in the care and treatment of people with frailty, including long-term conditions and dementia care experience.
