We are seeking a dedicated and compassionate Dementia Care Specialist to join our growing team at Hampshire Hospitals NHS Foundation Trust. You will play a vital role in providing exceptional care and support to patients living with dementia, working closely with their families and carers.
Your Responsibilities
- Deliver person-centered care to individuals diagnosed with dementia, ensuring their well-being and dignity are prioritized.
- Collaborate effectively with the multidisciplinary team to develop and implement individualized care plans.
- Maintain clear and open communication with patients, families, and carers, providing updates and addressing concerns.
- Contribute to a supportive and positive environment for patients and staff alike.
About You
The ideal candidate will possess:
- A minimum of one year of demonstrable experience in dementia care within a healthcare setting.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients and their loved ones.
- A genuine passion for providing compassionate and high-quality care to individuals living with dementia.
Qualifications & Training
- Dementia Level 2 training is essential.
- Additional dementia qualifications are desirable.
