We are seeking a highly skilled and compassionate Community Outreach Officer to join our team at Age UK East London. Key Responsibilities- Provide outreach support services to older adults, promoting independence and well-being.
- Develop and maintain relationships with statutory and voluntary sector agencies to ensure access to appropriate services.
- Work closely with service users to identify their needs and develop individual support plans.
- Signpost people to relevant services, promoting network services across the Boroughs.
- Collaborate with colleagues to develop shared working practices and improve service delivery.
- Contribute to the evaluation of services, collecting monitoring information to inform improvements.
- Provide support to services outside of normal working hours, as required.
- Experience of working with marginalized groups, providing one-to-one support and running group activities.
- Ability to keep accurate records and statistical data for monitoring purposes.
- Excellent communication and interpersonal skills, with a commitment to providing high-quality customer service.
- Intermediate IT skills and ability to use word-processing software.
- Commitment to learning and development, with a willingness to reflect on practice and improve services.
- A competitive salary, ranging from £28,840 to £31,930, plus a 5% employer pension contribution.
