Job Summary

We are seeking a highly skilled and compassionate Community Outreach Officer to join our team at Age UK East London. As a key member of our Community Outreach team, you will play a vital role in providing high-quality outreach support services to older adults with low-level support needs, enabling them to live independently within their communities.

Key Responsibilities
  • Provide outreach support services to older adults, promoting independence and well-being.
  • Develop and maintain relationships with statutory and voluntary sector agencies to ensure access to appropriate services.
  • Work closely with service users to identify their needs and develop individual support plans.
  • Signpost people to relevant services, promoting network services across the Boroughs.
  • Collaborate with colleagues to develop shared working practices and improve service delivery.
  • Contribute to the evaluation of services, collecting monitoring information to inform improvements.
  • Provide support to services outside of normal working hours, as required.
Requirements
  • Experience of working with marginalized groups, providing one-to-one support and running group activities.
  • Ability to keep accurate records and statistical data for monitoring purposes.
  • Excellent communication and interpersonal skills, with a commitment to providing high-quality customer service.
  • Intermediate IT skills and ability to use word-processing software.
  • Commitment to learning and development, with a willingness to reflect on practice and improve services.
