About the Role

We are seeking a highly skilled and experienced Care Home Practitioner to join our Crisis Assessment & Treatment Team at West London NHS Trust.

Key Responsibilities
  • Deliver the gatekeeping function for admission to inpatient mental health units and offer in-reach work in wards to facilitate discharges.
  • Plan, implement, and evaluate programmes of care for clients in crisis with acute mental health problems in the community, and coordinate activities with other professionals and agencies.
  • Offer interventions, advice, and support on the telephone and undertake first response face-to-face crisis assessments in a variety of settings.
  • Ensure that patients receive the most appropriate care and treatment following triage and assessment, referring to the most appropriate mental health provision or signposting to the relevant services.

Requirements
  • Post-qualification experience to include appropriate experience within an acute mental health care setting and/or significant experience within community, crisis care, or liaison psychiatry.
  • To understand and have experience of risk management.
  • Experience of receiving supervision.
Desirable
  • Advanced Assessment Skills training.
  • Experience with telephone triage within a Single Point of Access Service.
Knowledge & Skills
  • A high level of written English.
  • Knowledge of adult and children safeguarding requirements.
  • IT literate - specifically in the MS office suite of programmes.
Specific Requirements
  • Availability to work a shift rota covering 7 days a week 365 days a year.
  • Full UK driving licence.
