We are seeking a highly motivated and experienced pharmacist to join our team as a Rotational Senior Pharmacist. This is an exciting opportunity to work in a dynamic and award-winning department that puts clinical pharmacists at the forefront of medicines optimisation and patient care.
About the RoleThe successful candidate will work closely with medical and nursing staff and the wider multidisciplinary team on a wide range of medicines optimisation matters. They will be responsible for delivering quality care to patients at ward level through efficient medicines optimization, including the assessment of patient's own drugs, medicine reconciliation, original pack dispensing at ward level, counseling patients about their medicines, and coordinating the supply of discharge medication.
Key Responsibilities- Work on the wards and deliver quality care to patients at ward level through efficient medicines optimization.
- Assess patient's own drugs, medicine reconciliation, original pack dispensing at ward level, counseling patients about their medicines, and coordinating the supply of discharge medication.
- Work with the ward-based team to continually develop the service, while maintaining high standards and achieving best practice to meet the needs of patients and the Trust.
- Dispensary-based responsibilities.
- Post registration experience in an acute hospital-based clinical post.
- Experience working as part of a multi-disciplinary team to provide patient care.
- Experience of undertaking research, quality improvement, or audit projects.
- Experience delivering care in acute medicine and surgery specialties in a ward setting.

Salary: £51,488 to £57,802 per annum (inclusive of pay award pending)
