**Job Summary**

We are seeking two enthusiastic and highly motivated Community Care Officers to join our team at Nottinghamshire County Council. As a Community Care Officer, you will play a vital role in supporting adults aged 18 and over who are detained within one of three Nottinghamshire Prisons and approved premises.

**Key Responsibilities**

  • Adopt a strength-based approach to fulfilling our statutory obligations under The Care Act 2014 by linking and working directly with the prisons and approved premises.
  • Establish a principle of equivalent care to individuals, whether that be whilst still in prison, moving on into the community or another authority.
  • Have a good understanding of the relevant legal frameworks and the promotion of human rights.
  • Experience of working with individuals with multi-disadvantage is essential as is a good understanding of reablement and risk assessment/risk management.
  • Good communication skills and ability to work with people, family/carers, health colleagues and other key partners.

**Requirements**

  • Complete His Majesties Prison & Probation Service vetting alongside an enhanced DBS.
  • Be able to drive and have access to a vehicle.
