We are seeking a qualified Social Worker to join our Adult Locality Service team in South London. As a key member of our team, you will be responsible for providing exceptional care and support to vulnerable adults in need.
Key Responsibilities- Conduct comprehensive needs assessments and develop personalized care plans to ensure the safety and well-being of individuals in the community.
- Work collaboratively with service users, their families, and other professionals to provide person-centered solutions.
- Undertake safeguarding investigations to protect vulnerable adults from harm.
- Provide advice and advocacy to individuals, supporting them to access services and achieve greater independence.
- Manage a varied caseload effectively, ensuring timely interventions and reviews.
- Maintain accurate case records in line with legislative and organizational requirements.
- Work in partnership with health services, voluntary organizations, and other local authority departments to ensure a multi-agency approach to care.
- Contribute to service development by sharing best practices and participating in team meetings.
- Qualified Social Worker with a valid registration with Social Work England.
- Proven experience in working with adults in a social care setting.
- Knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act, and Safeguarding Adults frameworks.
- Excellent assessment, care planning, and case management skills.
- Strong communication and interpersonal skills, with the ability to work collaboratively with service users, families, and professionals.
- A proactive, solution-focused approach, with a passion for supporting individuals to achieve their full potential.
