**Reablement Care Role - £28,600 per year**Full-time position with a competitive salary and benefits package.

Care Outlook LTD is seeking an experienced and reliable healthcare professional to join our reablement service team in the Oxfordshire area, mainly Cherwell. As a Care Assistant Reablement Specialist, you will play a vital role in supporting vulnerable adults regain their independence following a stay in hospital.

Key responsibilities include:

  • Conducting assessments with new clients to identify their needs and develop personalized care plans.
  • Providing hands-on support with daily living tasks, such as personal care, toileting, meal preparation, and medication management.
  • Recording and reporting changes in clients' abilities and needs, ensuring seamless communication with the Home First team.
  • Building strong relationships with clients, colleagues, and the Home First team to deliver high-quality care.
  • Remaining adaptable and responsive to changes in clients' needs.
