Care Assistant Reablement Specialist

Supporting Vulnerable Adults in the Community

We are seeking experienced and reliable healthcare workers to join our reablement service in the Oxfordshire area, mainly in Cherwell. As a Care Assistant Reablement, you will play a vital role in supporting vulnerable adults in the community to regain their independence following a stay in hospital.

Key Responsibilities:
  • Conduct assessments with new clients following discharge from hospital to identify their needs and develop personalized care plans.
  • Support clients to regain independence with tasks such as personal care, mobility, and daily living activities.
  • Work collaboratively with clients, their families, and other healthcare professionals to ensure seamless care and support.
  • Travel to clients' homes, using your own vehicle, and receive paid mileage.
Requirements:
  • Hands-on experience working in a care setting, preferably in a reablement or rehabilitation role.
  • Confidence in following care plans and supporting clients with varying needs.
  • Ability to work independently and as part of a team.
  • Valid driving license and access to a car.
