About the Role

We are seeking a highly skilled and experienced Care Management Officer to join our team at NHS North West London Integrated Care Board. As a Designated Clinical Officer, you will play a crucial role in improving healthcare services and outcomes for children and young people with Special Educational Needs and Disabilities (SEND) in our region.

Key Responsibilities
  1. Provide leadership and expertise on matters relating to SEND for the ICB and Local Partnership Area, including advice for strategic commissioning.
  2. Support effective partnership working in local areas to deliver clear plans and improvements in care for children with SEND.
  3. Build partnerships and a shared understanding of the challenges and solutions as a key feature of this role.

As a Care Management Officer, you will work closely with our team to ensure that the outcomes for disabled children and young people and those with SEND are maximised. You will be responsible for providing support and advice on the framework and preparation of Education, Health and Care (EHC) plans, and ensuring that identification, assessment, and planning takes place, and that health support is available for children and young people undergoing EHC needs assessment and Annual Reviews of existing EHC plans.

You will also be responsible for providing advisory services on practical guidance and policies relating to SEND, and advising ICB senior managers on their statutory responsibilities and SEND reforms implementation. Additionally, you will work with the Head of SEND to devise, review, manage and update policies and procedures in relation to SEND for ICBs, ensuring that they reflect the current statutory guidance, and disseminate the policies and procedures throughout the ICB and local health economy.

Requirements
  • Qualified and registered Allied Health Professional or Registered Nurse or Specialist Community Public Health Nurse (child family) with substantial experience in SEND.
  • An in-depth and comprehensive understanding of the Children and Families Act, SEND Code of Practice, and statutory requirements of the ICB.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with stakeholders, including children, young people, families, and professionals.
  • Ability to work effectively in a fast-paced environment, prioritising tasks and managing multiple projects simultaneously.
  • Strong analytical and problem-solving skills, with the ability to interpret complex data and make informed decisions.
