About the Role

We are seeking a highly skilled and dedicated Mental Health Social Worker to join our team at 5 Star Recruitment. As a key member of our team, you will play a vital role in assessing and managing the complex needs of individuals with mental health issues and their carers.

Key Responsibilities
  • Assess and manage the complex needs of individuals with mental health issues and their carers to ensure their health, wellbeing, and social inclusion.
  • Liaise with professionals to ensure seamless care pathways that safeguard and promote independence.
  • Promote joint working to support person-centered outcomes for individuals with mental health issues.
  • Undertake safeguarding investigations as required.
  • Assist the Local Authority in meeting their statutory obligations under the Mental Health Measure and the Social Services and Wellbeing (Wales) Act 2014.
  • Deliver on the Local Authorities' statutory requirements within the MHA 1983.
Key Skills and Qualifications
  • Ability to assess and manage complex needs of individuals with mental health issues and their carers.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with other agencies.
  • Knowledge of relevant legislation and policies.
  • Ability to maintain accurate and up-to-date records.
