Job Summary:
We are seeking a highly motivated and enthusiastic Community Psychiatric Nurse to join our Crisis Community Mental Health Team in Newcastle and surrounding areas. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with mental health needs.
Key Responsibilities:
- Provide comprehensive assessments and care planning for individuals with mental health needs
- Develop and implement individualized care plans to promote recovery and well-being
- Collaborate with multidisciplinary teams to ensure seamless care coordination
- Provide education and support to individuals, families, and carers to promote mental health and well-being
- Participate in quality improvement initiatives to enhance service delivery and outcomes
Requirements:
- Registered Nurse with a valid nursing license
- Experience working in mental health services, preferably in a community setting
- Strong assessment and care planning skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
