Job Title: Advanced Healthcare Practitioner

Job Summary:

We are seeking an experienced Advanced Healthcare Practitioner to join our team at The Mid Yorkshire Teaching NHS Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality clinical care to patients with complex needs.

Key Responsibilities:
  • Assess, diagnose, and develop individualized management plans for patients with diverse presentations and complex symptoms.
  • Work autonomously in a clinic setting, assessing, diagnosing, planning, and implementing pathways of care for a highly specialist caseload.
  • Communicate effectively with patients and carers to maximize rehabilitation potential and ensure understanding of condition.
  • Use verbal and non-verbal communication skills to gain informed consent with patients, including those with barriers to effective communication.
  • Assess capacity, gain valid informed consent, and work within a legal framework with patients who may lack capacity to consent to treatment.
  • Lead therapy at MDT meetings, case conferences, and ensure therapeutic management of patients is integrated and rehabilitation potential is maximized.
  • Refer patients onwards to relevant healthcare professionals and ensure robust clinical governance for the service specialty.
  • Work extended hours and weekends as service requires, including emergency work as per service business continuity plan.

Requirements:

To be successful in this role, you will need to have:

  • Advanced clinical skills and knowledge in a specialist area.
  • Experience of working autonomously in a clinic setting.
  • Excellent communication and interpersonal skills.
  • Ability to work within a legal framework and assess capacity.
  • Strong leadership and teamworking skills.
