Job Title: Senior Social Worker for the Assessment and Safeguarding Team

As a key member of our Adult Assessment and Safeguarding Team, you will play a vital role in ensuring the safety and well-being of vulnerable adults. Your expertise will be instrumental in identifying risks and implementing effective safeguarding measures.

Key Responsibilities:
  • Conduct thorough assessments of vulnerable adults to identify their needs and risks
  • Develop and implement tailored care plans to ensure the safety and well-being of adults at risk
  • Collaborate with multidisciplinary teams, including individuals, families, and healthcare professionals, to provide comprehensive support
  • Identify and mitigate risks, ensuring the safety and well-being of vulnerable adults
Requirements:
  • Strong understanding of adult safeguarding principles and practices
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong analytical and problem-solving skills
