Job Opportunity

We are seeking a highly skilled and experienced Social Worker to join our Hospital Discharge Team on a temporary basis. This role is critical in supporting the safe and timely facilitation of hospital discharges, ensuring that patients receive the necessary support and care to transition smoothly from hospital to home or other appropriate settings.

Key Responsibilities:
  • Discharge Planning: Facilitate safe and timely hospital discharges by working closely with the Hospital Discharge Team and other healthcare professionals.
  • Caseload Management: Manage a complex caseload of adults, ensuring that assessments and care plans are completed in a timely manner to support discharge.
  • Person-Centred Assessments: Conduct person-centred assessments to determine the needs of individuals and their families, ensuring that discharge plans are tailored to meet identified needs.
  • Collaboration and Communication: Collaborate with multidisciplinary teams, including nurses, doctors, therapists, and community partners, to develop discharge plans that support patients' safe transition from hospital to home or other appropriate settings.
  • Service User Support: Provide professional guidance and support to service users and their families during the discharge process, ensuring that they are fully informed and involved in decision-making.
  • Record Keeping: Maintain accurate records and documentation using Eclipse (knowledge of Eclipse is desirable).
  • Compliance and Best Practice: Adhere to relevant legislation, policies, and procedures to ensure compliance and best practice in all discharge planning activities.
  • Flexibility and Adaptability: Respond promptly and effectively to changing priorities and pressures within the team, demonstrating the ability to work under pressure.
  • Travel: Drive to various locations as required to support patients and facilitate discharges.
Essential Criteria:
  • Qualifications and Experience: Qualified Social Worker with current registration with Social Work England and proven experience working within an Integrated Hospital Discharge Team.
  • Caseload Management: Ability to manage a complex caseload and make sound decisions quickly and effectively.
  • Communication and Organisational Skills: Excellent communication, assessment, and organisational skills, with a commitment to collaborative working.
  • Legislation and Policy Knowledge: Knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and Safeguarding Adults procedures.
  • IT Skills: Proficiency in using IT systems, including knowledge of Eclipse (desirable).
  • Driving License: Full UK driving license and access to a car for work purposes.
