Location:
FULLY ON SITE BASED-CROYDON 5 DAYS A WEEK. ALL APPLICANTS MUST HAVE A CLEAN DRIVING LICENSE AND OWN VEHICLE.

Role Purpose:
To deliver a needs-oriented support service to elderly individuals and those with physical disabilities or sensory impairments. This role will also encompass service users experiencing mental health challenges related to dementia, as well as older adults with functional mental health conditions and physical frailty.

The position involves dedicating time to a designated group of service users or referring those with more intricate needs, thereby fostering their recovery and sustaining their independence within the community.

Responsibilities include the installation, maintenance, and removal of telecare equipment within the homes of service users.

Additionally, the role entails providing an emergency response service or home visit service to all service users assessed as requiring such support, including assistance via telephone.

Furthermore, the role includes delivering an emergency response service to all service users connected to the Croydon Careline / Telecare Alarm system.


Key Accountabilities:


To visit service users as assessed and maintain regular contact with all service users as outlined in policies and procedures.


  • To ensure a clear written audit trail, in accordance with policies and procedures.
  • To keep service user records current, including conducting service reviews and updating the ARC with any new client details upon installation of telecare equipment or changes to existing client information. This includes notifying the ARC of any service users who have concluded services.
  • To document calls received and actions taken, ensuring that serious or unusual incidents are reported to the management team promptly.

Essential Knowledge:
Familiarity with resources available to adults requiring care and support.
Knowledge of relevant legislation, such as the Care Act 2014.
Ability to analyze and resolve situations effectively.
Commitment to empowering and enabling service users.
Capability to provide informed choices and support to users and caregivers.
