Key Responsibilities- Manage a complex and diverse caseload, working to prevent admission to the acute sector and supporting patients in their preferred place of care.
- Maintain detailed, accurate, and confidential patient records, adhering to Trust policies.
- Lead an effective supportive team, working together to provide clinical advice and support to other staff and patients.
- Manage a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programs.
