About the Role

We are seeking a knowledgeable and motivated Senior Occupational Therapist to join our multidisciplinary team in the Integrated Case Management service. As a key member of the team, you will work closely with other healthcare professionals to provide holistic assessments and treatment plans for patients in the community.

Key Responsibilities
  • Conduct initial assessments at home, performing a comprehensive evaluation of patients' needs and developing treatment plans in collaboration with consultants.
  • Work closely with the multidisciplinary team to provide evidence-based clinical interventions and promote patient choice.
  • Manage patients' care in the community, up to 6 weeks, with the aim of stabilizing their health and reducing hospital admissions.
  • Collaborate with community agencies, including community matrons, district nursing services, and in-patient therapy services.
Requirements
  • BSc degree or equivalent in Occupational Therapy.
  • Registration with the Health and Care Professions Council.
  • Evidence of continuous professional and personal development relevant to rotations.
  • Wide-ranging experience in musculoskeletal outpatients, adult/older people's rehabilitation, community rehabilitation, and neurology placements.
  • Registration with the Royal College of Occupational Therapy.
  • Evidence of leadership courses/training.
