We are seeking a skilled Social Worker to join our Hospital Discharge Team at Connect2Hampshire. As a key member of our team, you will play a vital role in supporting the turn-around of patients at the front door and working closely with the Single Point of Access.
Key Responsibilities- Collaborate with the acute team to ensure seamless patient care.
- Assess patients for discharge planning and develop effective strategies for their safe and timely discharge.
- Work on-site 4 days a week, assessing patients and developing discharge plans.
- 1 day a week will be spent working remotely, covering another hospital and providing support to the team.
- 3 years of post-qualifying experience in social work.
- Hold a social work degree and have a strong understanding of child protection legislation.
- Experience working with children and families in a social work setting.
- Knowledge of child protection issues and substitute care.
- A 9-day working fortnight.
- Annual leave of 25 days per annum.
- A personal assistant to provide back-office support and free up time for professional practice.
- A hybrid tablet device for flexible and effective working.
- 5 personal development days per annum.
- Access to permanent vacancies as they become available.
- Hybrid working, with 1 day a month in the office for development days.
- £38 per hour umbrella rate.
