The Forward Trust is seeking a skilled Health and Wellbeing Practitioner to join our team. As a Health and Wellbeing Practitioner, you will be responsible for delivering client-centered treatment to individuals with substance misuse and low-level mental health issues.
Key Responsibilities- Manage a caseload of clients and provide harm reduction advice and guidance
- Assess, plan, and deliver structured 1:1 and group-work sessions for substance misuse and low-level mental health service users
- Provide transitional support from prison to the community through integrated working with other Health and Wellbeing agencies
- Develop and contribute to the Substance Misuse and general Health and Wellbeing care needs of service users
- Facilitate/co-facilitate substance misuse and mental health groups and Health and Wellbeing programmes
- Knowledge of the issues facing substance misusers and low-level mental health issues
- Demonstratable experience of carrying out risk assessments, comprehensive assessments, and designing and implementing SMART care plans
- Significant experience of carrying out psychosocial interventions, including using motivational interviewing techniques in 1:1 and therapeutic group settings
- A thorough knowledge of and commitment to the 12-step process of recovery from addiction
- Experience of providing counselling/CBT services to prisoners or those with Health and Wellbeing or drug and/or alcohol problems
