We are seeking a dedicated Community Nursing Specialist to join our team, focusing on delivering exceptional healthcare services within the community.
This role involves a full-time commitment, with a permanent contract, and requires flexibility to cover various shift patterns throughout the week.
As part of our compassionate community nursing team, you will provide comprehensive care in patients' homes, ensuring a holistic approach to health and well-being.
An exciting opportunity exists within our Community Nursing Team, which is committed to providing safe, effective, and patient-centered care to our diverse patient population. We welcome applications from both experienced and newly qualified nurses who are enthusiastic about advancing their careers in community nursing.
Key Responsibilities
- Manage referrals and prioritize workload effectively.
- Conduct assessments and continuously evaluate risk management for patients, caregivers, and staff.
- Deliver and assess all aspects of clinical nursing care in accordance with best practices.
- Utilize medical equipment and technology to support patient assessments and treatment delivery.
- Perform a variety of clinical tasks, including venepuncture, wound care, catheterization, and tracheostomy management.
- Collect samples for diagnostic testing, including blood and wound swabs.
- Educate and support patients and caregivers to promote independence and rehabilitation.
- Provide dignified care for terminally ill patients, adhering to established care pathways.
- Maintain a thorough understanding of medications and their effects on various conditions.
- Oversee nurse prescribing practices to ensure cost-effective medication management.
- Administer medications while monitoring their effects on patients.
- Communicate effectively with patients and all stakeholders involved in care delivery.
- Manage the ordering and maintenance of specialized nursing equipment.
