As an Occupational Therapist Assistant, you will play a vital role in supporting vulnerable adults with physical disabilities, sensory impairment, mental health, and learning disabilities. Your primary responsibility will be to assess and manage an independent caseload, providing equipment, minor and major adaptations, and assistive technology to enable citizens to live independent lives.

Key Responsibilities:

* Assess and manage an independent caseload of vulnerable adults with physical disabilities, sensory impairment, mental health, and learning disabilities.
* Provide equipment, minor and major adaptations, and assistive technology to support citizens in their daily lives.
* Work closely with other members of the multi-disciplinary team to ensure comprehensive support planning and assessment.
* Facilitate and contribute to the assessment of the needs of vulnerable adults and their carers, feeding into the process of specialist and more complex assessments.
* Support citizens to promote their health, well-being, choice, and control.

Essential Qualifications and Experience:

* Demonstrate an understanding of the needs of vulnerable adults and their family/carers.
* Demonstrate an understanding of the concept of personalisation and self-directed support to enable citizens to live independent lives exercising choice and control in decision-making working in a re-abling manner.
