About the Role

We are seeking a highly motivated and experienced Community Practitioner to join our team in the Older Adults pathway. As a key member of our crisis team, you will be responsible for providing high-quality care and support to service users with complex needs.

Key Responsibilities
  • Assess and manage casework of service users with substantial and complex needs.
  • Provide specialist clinical advice to service users, carers, staff, and statutory agencies.
  • Develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews and adaptations as necessary.
  • Consider carer needs and involve carers and family in care planning where appropriate.
  • Work collaboratively with the multi-disciplinary team to deliver high-quality individualized care.
  • Coordinate treatment planning, interventions, reviews, and discharge planning.
Requirements
  • First-level registered nurse (RNMH, RMN, RNLD, or equivalent) or social worker with post-registration training in clinical/psychological interventions.
  • Proven track record of post-registration clinical experience.
  • 12-24 months of band 5 qualified nurse experience.
  • Mentor training (desirable).
Desirable Skills
  • Understanding of professional accountability, NMC standards, and local/national initiatives.
  • Understanding of difficult-to-manage/challenging behaviors.
  • Proven ability to complete comprehensive assessments of complex mental health needs.
  • Risk assessment and management skills.
  • High standard of written and verbal communication.
  • Ability to work under pressure.
  • Clinical leadership and delegation skills.
