About the Role

We are seeking a highly skilled and dedicated Community Nurse Practitioner to join our team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient care and support to individuals with complex health needs.

Key Responsibilities
  • Assess and manage a caseload of patients with substantial and complex needs, who are at risk of hospital admission, placement breakdown, and presenting as acutely unwell.
  • Provide responsive and intensive specialist clinical advice to patients, families, carers, staff, statutory agencies, and third sector partner agencies.
  • Develop and implement care plans, safety plans, and recovery and wellbeing plans in collaboration with patients and their families.
  • Coordinate treatment planning, interventions, clinical reviews, and discharge planning.
  • Provide clinical supervision to staff as delegated by Team Manager and Clinical Leads.
Requirements
  • Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
  • Mentor Training.
  • Evidence of post-registration training in clinical interventions/skills relevant to the post.
  • Proven track record of post-registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi-Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.
Desirable Qualifications and Skills
  • Experience of working in a community team.
  • Experience of crisis management.
  • Clinical leadership.
  • Experience/Understanding of care and treatment reviews.
